Course Waiver/Substitution
Request for Course Waiver/Substitution Career and Technical Education Programs:
- Students must see a counselor to submit the Request for Course Waiver/Substitution form [PDF].
- Counselor will do a credit/progress check and make sure the substitute course is appropriate (counselors can check the waiver/sub log, etc.) and not being used to satisfy another requirement.
- Counselor and student will attach all relevant documents including transcripts and/or UH advising documents and course description if necessary to each request. This will eliminate delays when requests need to be routed to Division Chairs of different academic areas.
- Counselor and student will both sign the form.
- A log of all outgoing forms will be maintained in Counseling. Dean's secretaries will be asked to routinely report on which forms have been received in their offices. Their report will be compared to the Counseling log and the program division chairs will be asked to follow up on forms that have not been completed in two weeks.
- Counselor submits to Discipline Curriculum Liaison.
- Once approved, form will be given to Counselor who signed the form. Counselor will process transfer courses if applicable.
- Counselor will give to Records Office for recording and notification of student and counselor.
